Registration & Conference Fees
Registration form can be downloaded HERE (.doc) or you can order it via e-mail: email@example.com .
Fully completed registration form and confirmation about deposit must be send to e-mail: firstname.lastname@example.org .
A letter of confirmation/payment receipt will be sent by email. Attendees must present this confirmation/payment receipt at the registration counter as a proof of their registration and payment .
Early registration (until 1st of July)
130 EUR for medical seminar
80 EUR for allied health profesionals and residents
50 EUR for lay seminar
Late registration (from 1st of July)
150 EUR medical seminar
100 EUR allied health profesionals and residents
70 EUR for lay seminar
One day conference fee is 70 EUR.
Price includes conference materials, coffe breaks, lunch and Saturday dinner.

Registration Cancellation Policy
Notification of cancellation must be made in writing and sent to the Croatian Cystic Fibrosis Association via e-mail or regular post. The notification must include all the relevant information regarding the bank account to which a possible refund may be remitted.
If the written notification of cancellation for a booked registration is received before the early registration deadline (1 July 2010), a full refund minus a handling fee of 30 Euro will be made. No refunds will be made for cancellations received after 1 July 2010. All refunds will be made after the conference.
